Applicant is required to provide a valid and current medical examiner's certificate, documents proving legal name, date of birth, legal presence, social security number, and 2 documents for proof of resident address. If you are using a post office box, 1 address document should be with post office box and 1 with physical address. Address documents must be from different entities.

If you have lost your CDL CLASS A, B, or C, you must provide documents to identify yourself. If you are changing your name or address, you must provide name change document (marriage certificate, divorce decree, etc.) and 2 documents for proof of resident address. If you use a post office box as a mailing address, bring 1 document with the post office box address and 1 with your physical address. Address documents must be from different entities.

We require your out of state CDL CLASS A, B, or C (CDL MUST BE PRINTED ON YOUR LICENSE), medical examiner's certificate, documents proving legal name, date of birth, legal presence, social security number and 2 documents for proof of resident address. You must have a clear driving status on the National Drivers Registry. If you are holding a Hazardous Materials endorsement you are required to take a knowledge test.

We require a Hawai'i driver's license to take any of the CDL knowledge tests. Tests include, General Knowledge, Special Knowledge Tests - Air Brakes, Combination Vehicles, Doubles & Triples, Tanker, Hazardous Materials, Passenger Bus, School Bus. You are required to successfully complete the General Knowledge test before taking any of the Special Knowledge Tests.

We require a Hawai'i driver's license, documents proving legal name, date of birth, legal presence, social security number and 2 documents for proof of principal address. If you are using a post office box for your mailing address, bring 1 document with the post office box and 1 with your physical address. A current and valid medical examiner's certificate is required. If you need to take a knowledge test, please make an appointment under CDL Knowledge Test.
